Group Home Referral Form

At the Youth Crisis Center, Inc. (YCC), our Group Home program is designed to foster growth and transformation by integrating residents, families, community support networks, and the Department of Family Services (DFS). Together, we develop comprehensive case plans that provide abundant opportunities for personal and familial development. Guided by a dedicated on-site case manager, each youth receives weekly support to facilitate the achievement of their goals.

To encourage proactive engagement with the program, residents can earn tokens for positive actions. Staff organize outings and encourage participation in community activities, promoting social integration and skill-building. The Youth Advisory Panel (YAP) provides a platform for residents to voice their ideas and preferences, empowering them to shape their living environment positively.

Residents also participate in weekly cooking lessons and therapeutic groups, and community services, acquiring essential life skills.

What to Expect From Our Group Home

Upon acceptance into the group home program, youth will be required to participate in individual and family counseling. YCC provides clinical staff for these services, but families may also choose providers within the community. Maintaining contact with any clinical providers is essential for the successful completion of the program. Youth and families will undergo a start-up process, have weekly communication with the case management team, and regular meetings will be held to discuss youth progress and update case plans and goals. These services will include regular check-ins to assess needs and provide support. The YCC Group Home Case Management team will offer aftercare services for at least one year following exit from the program. The length of stay in the program is determined by the youth’s progress and needs.

Admission Criteria

Youth Crisis Center has the right to choose who will be served within the program. This decision is based on multiple factors, including youth needs and the program’s ability to meet those needs. YCC does not discriminate against any person on the basis of race, ethnicity, national origin, color, sex, sexual orientation, gender identity or expression, or religion. The YCC Group Home offers long-term care for children ages 10-17 years who are unable to return home at the present time. Youth accepted into the program must be recipients of Medicaid. Each referral is reviewed on an individual basis.

Family Involvement

When possible family involvement is helpful to the success of individuals in the YCC Group Home program and beyond. Families play an active role by attending monthly regular meetings and participating in family counseling sessions. They are encouraged to communicate with the case manager regarding their child’s behavior, and can reach out anytime. Families can also request additional services or referrals, provide input progress and the appropriateness of consequences, and attend family events at YCC or within the community. Celebrating achievements is also important, and families are invited to attend the departure party to honor the accomplishments of the youth in the program.
